Decision title

Decision#

State the choice and its scope. Keep the status proposed until the source or an authorized person accepts it.

Evidence#

Add the source link and record ID. Include the supporting passage location. Use a heading or paragraph when a timestamp is unavailable.

Reason and alternatives#

State the recorded reason. Distinguish it from an inferred reason.

Unresolved details#

Name unknown dates, owners, dependencies, or approvals.

Link the earlier decision and affected tasks. Mark a replaced decision as superseded. Preserve its source.
